Health History Questionnaire

Please complete this form before your appointment. You can fill it out online and submit it directly to our patient concierge, or print the form and complete prior to your appointment then bring with you to your first appointment. (A printed version is included with your mailed appointment confirmation packet.) When you arrive, let us know if you completed it online or have the completed from with you.

Your Medical Records

Prior to your appointment, please obtain copies of all your medical records pertaining to your cancer diagnosis from your physician’s office, hospital and/or clinic. This includes any pathology slides and CD/DVD copies of all completed MRI, CT, and ultrasound scans.

Once you have collected all of your pertinent medical records, please FedEx all of this information, except the pathology slides, to the address below. Due to the delicate nature of pathology slides, please bring these slides with you on your appointment date.

We would prefer to receive your medical records 24 - 48 hours prior to your appointment with us. However, if you are unable to FedEx your records within this time frame, you may fax them to 313-576-9827 or bring them with you to your appointment.

Directions to Karmanos

From Windsor Tunnel

  • After you exit the tunnel and go through customs, turn right onto East Jefferson Ave.
  • Stay in left lane and merge onto North I-75
  • Continue on North I-75 to exit 52, Mack Ave.
  • Turn left onto Mack Ave. and continue for several blocks
  • Turn right onto John R.
  • Turn right at the first traffic light (about 2 blocks) then turn left into the Karmanos valet parking
  • Valet parking is available at the Karmanos main entrance, 4100 John R

What to Expect at Your Appointment

Plan to arrive 30 minutes prior to your scheduled appointment to complete the registration process.

Please use our valet parking, a complementary valet pass will be included in your appointment confirmation packet.

Check in at the information desk located in the main lobby. A representative from Karmanos will notify the Patient Service Representative of your arrival.

You will meet with a Patient Service Representative to complete the registration process and handle your payment.

When completed, a Patient Navigator will escort you to the check-in area for your appointment. You will be asked to be asked to take a seat in the waiting area until you are called into an exam room.

Once escorted to the exam room, your height, weight, blood pressure, heart rate, temperature and respiratory rate will be taken by a staff member.

A nurse, nurse practitioner or physician’s assistant will review your health history with you and may ask additional questions to obtain necessary information for the physician.

Following your meeting with the physician further tests or a treatment plan may be recommended. 

Frequently Asked Questions

What is the cost of the initial consultation visit?

The cost for the initial consultation at Karmanos Cancer Institute is generally $500. There may additional fees during the visit based on the level of services needed.  

Is it safe in your area of Detroit?

Absolutely! We are on the campus of the Detroit Medical Center, the largest medical center in Detroit, with seven hospitals on our main campus. We have an outstanding Security Team that provides 24-hour surveillance of our campus, and the surrounding neighborhoods.

Will you provide a letter for Immigration to indicate that I have an appointment at Karmanos? 

Yes, we can provide a letter for this purpose. Please ask the Patient Concierge Representative for this letter when you are scheduling your appointment.

How should I prepare to pay for my first appointment?

You may pay for your visit by cash or credit card. We accept Visa or MasterCard.

When will I get the formal report about this appointment? Will I get any information that day?

The physician(s) will discuss your diagnosis and treatment plan verbally with you during your appointment. You may also request a copy of the physician’s dictation following your appointment.

Is there someone that will meet me and help me get around there?

Yes, we can schedule a volunteer navigator to assist you during your first appointment.

How long will I be there on the first day?

Please plan on being at the hospital for 4-8 hours. You will be meeting with numerous members of our multidisciplinary team during your first visit. In addition, you may be asked to complete additional testing. It is recommended to bring reading materials or other devices to keep you occupied. Karmanos offers free Wi-Fi for our patients.  

Is there anywhere to eat at Karmanos?

Yes – following your registration, you will be given a beeper. You can enjoy refreshments or a meal at any of our on-campus eateries.
